| Abstract: | Práce se zabývá finanční nákladovou analýzou systému veřejné zdravotní péče při léčbě nemocných se srdečním selháním. Na základě aktuálních dat a spoluprací s odborníky z I. interní kardioangiologické kliniky Fakultní nemocnice Hradec Králové popisuje systémové výdaje na jejich léčbu. Detailně analyzuje farmakologickou léčbu podle nejnovějších doporučení České kardiologické společnosti i European Society of Cardiology. V návaznosti popisuje nákladovost nutných průběžných ambulantních vyšetření spojených s léčením pacientů se srdečním selháním v případě centralizované péče. Celkové náklady jsou přepočítány podle sledovaného vývoje cen v letech 20072017. Zjištěné hodnoty byly použity k vyčíslení dodatečných finančních prostředků v rámci veřejné zdravotní péče k zřízení první specializované ambulance pro léčbu srdečního selhání ve spádové oblasti Hradce Králové. Dle výsledků činnosti podobných specializovaných pracovišť ve světě i v ČR jsou určeny možné úspory v nákladech na hospitalizační léčbu. Práce poukazuje na možnost efektivnějšího, ekonomicky méně náročného, a zároveň pro pacienty vstřícnějšího způsobu léčby srdečního selhání v případě zřízení této specializované kardiologické ordinace. |

| Abstract: | The thesis deals with the financial cost analysis of the system of public health care in the treatment of patients with heart failure. Based on current data and with cooperation of experts from the 1st Department of Internal Medicine - Cardioangiology, University Hospital Hradec Králové describes system expenditures on their treatment. It also analyzes in detail the pharmacological treatment according to the latest recommendations of the Czech Society of Cardiology and the European Society of Cardiology. This thesis then describes the cost of the necessary ongoing outpatient examinations associated with the treatment of patients with heart failure. Total costs are recalculated according to the observed price developments in 2007-2017. The identified values were used to quantify additional funds in public health care to establish the first specialized ambulance for the treatment of heart failure in the catchment area of Hradec Králové. According to the results of similar specialized workplaces around the world and in the Czech Republic, possible savings in the costs of hospitalization are determined. The paper highlights the possibility of a more efficient, patient-friendly and economically less demanding treatment of heart failure in the setting up of this specialized cardiology ambulance. |
