Alzheimer disease in the Czech Republic – development of the amount of deceased and hospitalized

| Abstract: | Diplomová práce si klade za cíl nastínit současnou situaci úmrtnosti a hospitalizace pacientů v důsledku Alzheimerovy choroby. Dále s využitím dekompozice úmrtnosti zjistit, zdali vyšší počet zemřelých žen je způsobený vyšší intenzitou úmrtnosti žen, nebo tato skutečnost je způsobena věkovou strukturou. V neposlední řadě pomocí projekce s využitím Lee-Carterova modelu nastínit budoucí předpokládaný vývoj počtu zemřelých a hospitalizovaných pacientů s tímto onemocněním a následně provést projekci počtu hospitalizovaných pacientů podle druhu zdravotnického zařízení. Práce přináší základní historický přehled o vývoji Alzheimerovy choroby se zaměřením na stárnutí, neboť tato choroba se objevuje nejčastěji právě ve vyšším věku osob. Dále se zabývá zdravotní péčí v souvislosti s Alzheimerovou demencí včetně jejího vymezení v 10. revizi Mezinárodní klasifikace nemocí a přidružených zdravotních problémů. Popsán je rovněž i Lee-Carterův model, který je použit pro tvorbu projekcí. Pomocí analýz zhodnocujících současný vývoj bylo zjištěno, že s vyšším věkem se zvyšují počty hospitalizovaných pacientů ve zdravotnických zařízeních a dochází rovněž k růstu úmrtnosti v důsledku Alzheimerovy choroby. Dále bylo zjištěno, že rozdílnost v úmrtnosti mezi muži a ženami je způsobena především věkovou strukturou. Na základě projekcí lze do roku 2050 očekávat výrazné zvýšení počtu hospitalizovaných a zemřelých v důsledku tohoto onemocnění, kdy nejvyšší nárůst je očekáván ve věkové skupině 85 a víceletých osob, a to z obou zkoumaných hledisek. Nadále lze předpokládat, že nejvyšší počet hospitalizovaných pacientů ve zdravotnických zařízeních v důsledku Alzheimerovy demence bude léčen právě v psychiatrických léčebnách. |

| Abstract: | The main aim of the diploma thesis is to outline the present situation of the mortality and hospitalization of the patients because of Alzheimer disease. The next aim using decomposition of the mortality is to find out if higher amount of deceased women is caused with higher rate of mortality of women or this fact is caused with age structure. Last but not least I want to outline using projection and the Lee-Carter model the future expected development of the amount of deceased and hospitalized patients with this disease and then perform the projection of the amount of hospitalized patients according to the type of the medical facility. The thesis gets the basic historical overview about the development of Alzheimer disease focused on aging because this disease appears mostly in the higher age of the people. Next it deals with health care in connection to Alzheimer dementia including its specification in the 10th revision of the International Classification of Diseases and Associated Health Problems. There is also the description of the Lee-Carter model that is used for the creation of the projections. It was found out using the analyses evaluating the present development that there are with higher age rising numbers of hospitalized patients in medical facilities and there is also an increase of the mortality because of Alzheimer disease. Then there was discovered that the difference in mortality between men and women is caused mostly by the age structure. Based on the projections we can expect until the year 2050 significant rise of the number of hospitalized and deceased people because of this disease when the highest increase is being expected in the age group of 85 and perennial people and that is from the both of points of view. Then we can expect that the highest amount of hospitalized patients in medical facilities because of Alzheimer dementia will be treated just in psychiatric medical institutions. |
