Management and cost analysis of type 2 diabetes mellitus for HTA needs in Slovakia

| Abstract: | Diabetes mellitus 2. typu přináší zdravotní, ekonomické a sociální dopady. Počet diabetiků roste a s ním rostou také zdravotnické výdaje na toto onemocnění. Teoretická část práce zahrnuje rešerši aktuální literatury týkající se především epidemiologie diabetes mellitus 2. typu ve světě a na Slovensku, terapeutických postupů a standardů, ekonomické zátěže tohoto onemocnění a hodnocení zdravotnických technologií. Výzkum diplomové práce byl zaměřen na analýzu výdeje léků, dietetických potravin a zdravotnických pomůcek, uhrazených na základě veřejného zdravotního pojištění zdravotními pojišťovnami v souvislosti s DM2 na Slovensku, a analýzu ústavní zdravotní péče pojištěnců s diabetem mellitem 2. typu na Slovensku. Pro výzkum byly využity databáze publikované Institutem zdravotní politiky. Výsledky analýzy prokázaly narůstající počet pojištěnců s DM2, kterým byly vydány léky, dietetické potraviny a zdravotnické pomůcky, i těch, kteří byli v ústavní zdravotní péči. Rostoucí trend prokázala v analyzovaném období úhrada ústavní péče zdravotními pojišťovnami, stejně tak náklady na léky, dietetické potraviny a zdravotnické pomůcky. Výsledky dokumentují reálné využívání zdravotní péče u pojištěnců s DM2 a mohou posloužit při hodnocení nákladovosti a nákladové efektivnosti zdravotnických technologií či při rozhodovacích procesech v oblasti managementu DM2 na Slovensku. |

| Abstract: | Diabetes mellitus type 2 has significant health, economic and social impacts. The number of diabetic patients is on a rise and health expenses associated with this disease are increasing too. The theoretical part of this thesis includes a review of current scientific literature concerning epidemiology of diabetes mellitus type 2 in the world and in Slovakia, therapeutic procedures and standards, economic burden of this disease and health technology assessment. The research part of the thesis focused on the analysis of the dispensing of drugs, dietetic foods and medical aids, paid on the basis of public health insurance by health insurance companies in association with diabetes mellitus type 2 in Slovakia, and the analysis of institutional health care of insured persons with diabetes mellitus type 2 in Slovakia. Data for the research was derived from databases published by the Institute of Health Policy. The results of the analysis showed an increasing number of diabetes mellitus type 2 insured individuals who were prescribed medicines, dietetic foods and medical aids, as well as those who were in institutional care. In the analysed period, the reimbursement of institutional care by health insurance companies, as well as the cost of medicines, dietetic foods and medical aids, showed an increasing trend. The results document the real use of health care by insured patients with diabetes mellitus type 2 and can serve to assess the cost and cost-effectiveness of health technologies or in decision-making processes in management regarding diabetes mellitus type 2 in Slovakia. |
