| Abstract: | Disertační práce přináší komplexní pohled na problematiku finanční ochrany ve zdravotnictví v kontextu New Public Managementu. V rámci teorie staví disertační práce na podstatě konceptu New Public Managementu a na rámcích cílů a nástrojů zdravotní politiky. Finanční ochrana je představena jako jeden ze tří finálních cílů zdravotní politiky. Právě pohled přes finální cíle umožňuje ukotvení finanční ochrany v New Public Managementu. Práce klade důraz na představení metrik finanční ochrany (dle metodiky WHO) a následně je aplikuje do reálií ČR. Klíčovým vědeckým přínosem je vytvoření alternativní metodiky pro měření finanční ochrany, které součástí jsou 4 hranice pro určení katastrofických výdajů. Metodika WHO i alternativní metodika jsou testovány na datech z výběrového šetření výdajů domácností v letech 2007 až 2012. Právě toto období umožňuje sledovat zavedení regulačních poplatků ve zdravotnictví. Práce tak přináší nové evidence jejich dopadů na rozpočty domácností v ČR. Navazuje na přístupy Světové zdravotnické organizace, čímž jsou výsledky nejen vysoce validní, ale i vysoce relevantní pro tvorbu zdravotní politiky v ČR. Výsledky pro ČR naznačují, že ve sledovaném období má ČR vysokou míru finanční ochrany obyvatelstva, a to i po zavedení regulačních poplatků. Z analýzy vyplynulo, že největší zátěž pro domácnosti představují doplatky za léky. |

| Abstract: | The dissertation thesis brings a comprehensive view of the issue of financial protection in health care in the context of New Public Management. The theory builds on the essence of the New Public Management concept and on the frameworks of health policy goals and tools. Financial protection is presented as one of the three final health policy goals. Perception through the final goals allows to anchor financial protection in New Public Management. The thesis emphasizes the introduction of financial protection metrics (according to WHO methodology) and then applies them to the Czech Republic facts. A key scientific asset is the creation of an alternative methodology for measuring financial protection, which includes 4 limits for identifying catastrophic spending. WHO methodology and alternative methodology are tested on data from households sample survey from 2007 to 2012. It is this period that allows monitoring the introduction of regulatory fees in health care. The work brings a new evidence of their impact on household budgets in the Czech Republic. It builds on the approaches of the World Health Organization, making the results not only highly valid but also highly relevant to health policy making in the Czech Republic. The results for the Czech Republic indicate that in the period under review, the Czech Republic has a high level of financial protection for the population, even after the introduction of regulatory fees. The analysis showed that the biggest burden for households are the co-payments for medicines. |
