| Abstract: | Diplomová práce řeší účelově vázaný příspěvek na krizovou připravenost zdravotnických záchranných služeb z rozpočtu Ministerstva zdravotnictví ČR v letech 2013-2020. Cílem je zhodnotit stávající stav a analyzovat reálné potřeby praxe a tyto zjištěné skutečnosti dát do souvislostí. Teoretická část čtenáře uvádí do problematiky veřejného sektoru, do kterého je následně zasazeno zdravotnictví. Dále vysvětluje ekonomickou podstatu poskytování zdravotní péče, zabývá se zdravotnickými systémy a systémem zdravotnictví v České republice. Do něho zasazuje postavení, fungování a financování zdravotnické záchranné služby. Analytická část je rozdělena na 3 celky. Nejprve je analyzován vývoj přidělovaného příspěvku ve sledovaném období s ohledem na dopad na výdaje státního rozpočtu. Dále je pomocí polostrukturovaných rozhovorů se zástupci krizového managementu ze čtyř náhodně vybraných zdravotnických záchranných služeb identifikováno metodami otevřeného a axiálního kódování šest problematických oblastí v systému poskytování dotace. Na základě těchto okruhů je stanoveno šest hypotéz, které jsou ověřovány v poslední výzkumné části pomocí anonymního dotazníkového šetření. Toho se zúčastnili krizoví manažeři všech krajských zdravotnických záchranných služeb. Jedná se tak o úplný sběr dat. Výsledky odhalují slabé stránky a nedostatky v metodice pro přidělování finančních prostředků na krizovou připravenost ZZS. V závěru práce jsou navržena opatření v kontextu potřeb praxe. Týkají se zejména metodiky výpočtu částky a výše příspěvku. Navrhované změny by měly zvýšit akceschopnost a posílit krizovou připravenost zdravotnických záchranných služeb v celé ČR, zejména pak v méně zalidněných krajích. |

| Abstract: | This thesis looks at the Czech Ministry of Health budget’s ring-fenced contribution towards the crisis preparedness of the emergency medical services for 2013-2020. Its objective was to evaluate the current state and analyse the actual needs in practice, and put the facts thus ascertained into context. The theoretical section introduces readers to the issue of the public sector, into which the health service is then set. It then explains the economic substance of providing medical care, and looks at healthcare systems and the healthcare system in the Czech Republic. It then places the status, operation and funding of the emergency medical service in this context. The analytical section is divided into 3 parts. First of all, the development of the ring-fenced contribution over the period focused on is analysed, with its impact on state budget expenditure. Then semi-structured interviews with representatives of crisis management from four randomly selected emergency medical service providers are used to identify six problematic areas within the grant provision system using open and axial coding methods. On this basis, six hypotheses are posited, and these are tested in the final research section using an anonymous questionnaire survey. Crisis managers from all the regional emergency medical services took part in this survey, thus representing complete data collection. The results of this revealed weaknesses and shortcomings in the method of assigning funds for the crisis preparedness of emergency medical services. The study’s conclusion proposes measures in the context of practical needs. These mainly relate to methods of calculating the amount and level of contributions. The proposed changes should increase the capacity to act and strengthen the crisis preparedness of the emergency medical services throughout the Czech Republic, especially in less populated regions. |
