The Payment for State Insured Persons and Its Importance in Healthcare Financing in the Czech Republic: Current State and Future Perspectives

| Abstract: | Disertační práce se zabývá významem platby státu za státní pojištěnce ve financování zdravotnictví v České republice, která je spolu s vybraným pojistným klíčovou součástí příjmové strany systému veřejného zdravotního pojištění. Platba státu za státní pojištěnce je zasazena do teoretického, metodického i mezinárodního kontextu. Nastavení jejích konstrukčních prvků je klíčové ve změnách vyplacené částky ze státního rozpočtu. Pro podchycení mezinárodního kontextu byly na základě podobnosti s českým systémem financování zdravotnictví vybrány Belgie, Estonsko, Francie, Chorvatsko, Litva, Lucembursko, Maďarsko, Německo, Polsko, Rumunsko a Slovensko. Z analytického pohledu je hledán vhodný mechanismus pro valorizaci tohoto veřejného výdaje, kde byla uvažována široká škála přístupů - zvyšování platby absolutním či relativním meziročním přírůstkem platby, konstantním zastoupením platby na výdajích státního rozpočtu nebo na příjmech zdravotnictví, podíl na HDP, růst HDP, podíl na HND, růst objemu mezd a platů, podíl na minimální mzdě či všeobecném vyměřovacím základu, růst cenové hladiny, vazba na výnosy vybraných daní. Za využití ekonometrických metod je dále predikována platba státu v roce 2023, která by se dle modelu měla pohybovat mezi 135,82 a 136,46 mld. Kč. Z provedených analýz vyplynulo, že změna celkové úrovně platby státu za státní pojištěnce závisí více na změně vyměřovacího základu než na změně počtu státních pojištěnců. Počet státních pojištěnců je ovlivněn vývojem nezaměstnanosti, vyplacenými sociálními dávkami (bez důchodů) a počtem vězňů. Práce je využitelná pro praxi Ministerstva financí ČR, zdravotních pojišťoven a poskytovatelů zdravotních služeb. |

| Abstract: | The dissertation deals with the importance of the payment for state insured persons in the financing of health care in the Czech Republic, which together with the collected insurance premiums represent a key part of the income side of Czech public health insurance system. The payment for state insured persons is set into theoretical, methodological and international context. The setting of its structural elements reflects the key factor of changes of the total sumpaid from the state budget. To set up the international context Belgium, Estonia, France, Croatia, Lithuania, Luxembourg, Hungary, Germany, Poland, Romania and Slovakia were selected on the base of their similarity to the Czech healthcare financing system. From an analytical point of view a suitable valorization mechanism for this public expenditure is sought and a wide range of approaches is considered, namely increasing the payment by an absolute or relative year-on-year increase, constant share of the payment in state budget expenditures or in healthcare system revenues, the share of GDP, the growth of GDP, the share of GNI, the growth of the volume of wages and salaries, the share of the minimum wage or general assessment base, the growth in the price level, the share of collected tax revenues. The payment for state insured persons in 2023 is predicted by applying econometric methods. According to the model the payment should fluctuate between 135,82 and 136,46 billion CZK. The analyses confirm that the change in the total level of payment depends more on the change in the assessment base than on the change in the number of state insured persons. The number of state insured persons is affected by development of unemployment, paid social benefits (excluding pensions) and the number of prisoners. The thesis can be usable by the Ministry of Finance of the Czech Republic, health insurance companies and health service providers in case of their practical needs. |
