| Abstract: | Tématem předkládané práce je problematika návaznosti zdravotní a sociální péče při hospitalizaci pacienta a při jejím ukončování. V provozu zdravotnických zařízení dochází z různých důvodů k překročení doby hospitalizace nad dobu nutnou pro samotný léčebný proces, které lze kategorizovat jako sociální hospitalizace. V první části práce je provedena rešerše pro oblast péče o seniory, organizace zdravotních a sociálních služeb v České republice, doplněna o integrovanou péči a srovnání se zahraničím. Na teoretická východiska navazuje kombinovaný výzkum z oblasti zdravotnictví, který zahrnuje rozhovory se zaměstnanci zdravotnického zařízení a kvantitativní analýzu vlivu sociálních faktorů na délku hospitalizace a využívání zdravotnických zdrojů. Cílem práce je identifikovat charakteristiky sociálních hospitalizací a navrhnout opatření ke zlepšení návaznosti zdravotní a sociální péče. |

| Abstract: | The thesis explores the continuity between healthcare and social care during patient hospitalization and discharge. Due to various factors, patients may remain in healthcare inpatient facilities longer than treatment itself requires, a situation known as social hospitalization. The first part of the work consists of research in the field of elderly care, the organization of healthcare and social services in the Czech Republic, supplemented by integrated care and comparison with foreign countries. The theoretical background is followed by a combined research approach in the healthcare field, including interviews with hospital employees and quantitative analysis of the impact of social factors on the length of stay and the utilization of healthcare resources. The aim of the work is to identify the characteristics of social hospitalizations and propose measures to improve the continuity of health and social care. |
