The aim is to analyse the differences and similarities of the systems, cost-effectiveness and to determine which of the established systems for the procurement (and associated pricing) of pharmaceuticals is more cost-effective. For the purpose of the comparison, an analysis of tenders by the Danish state purchaser Amgros and a calculation of the average discount received when purchasing pharmaceuticals for Danish hospitals was carried out. Subsequently, an analysis of a selected group of pharmaceuticals based on Czech maximum prices compared to Danish two-week tenders. Finally, an analysis of the purchases of the General University Hospital in Prague (VFN) compared to the reimbursement less the calculated average discount received by Amgros. The paper takes into account factors such as the role of regulatory institutions, pharmaceutical reimbursement processes, the degree of centralisation and digitalisation, highlighting the key advantages and disadvantages of the systems analysed. It also includes an analysis of VFN spending, demonstrating how different systems affect the cost-effectiveness of their purchases. Finally, the thesis proposes recommendations for optimising the public pharmaceutical procurement system in the Czech Republic, based on inspiration from Danish practice, supported by data from the analyses. |
